QUESTION: Is the AHAI High School Participation Form still a required document? – A High School Hockey Club President
ANSWER: No, the AHAI High School Participation Form has not been a required document for approximately six (6) years; it is provided to High School clubs only as a convenience. There is no requirement for a physician’s approval to play, however, the Hold Harmless and the Consent to Treat forms are good for High School clubs, teams and/or organizations to have. The first protects the team/organization, and the second is best for the player should anything happen when the parent and/or guardian is not available.
