Youth Sports Safety Questionnaire

This survey is part of a research project attempting to understand how to best educate parents and coaches on various topics related to youth sports safety.

Although you will not get personal benefit from taking part in this research study, your responses may help us understand about how to provide education on health and safety topics to the parents and coaches of youth athletes.

We hope to receive completed surveys from about 200 people, so your answers are important to us. Of course, you have a choice about whether or not to complete the survey, but if you do participate, you are free to skip any questions or discontinue at any time.

The survey will take about 5-10 minutes to complete.

There are no known risks to participating in this study.

Your response to the survey is anonymous which mean no names will be used on research documents, or be used in presentations or publications. The research team will not know that any information you provided came from you, not even whether you participated in the study.

If you have any questions about the study, please feel free to ask; my contact information is given below. If you have any questions about your rights as a research participant or want to report any problems or complaints, you can call the Medical College of Wisconsin/Froedert Hospital Research Advocate at (414)456-8844.

Thank you in advance for your assistance with this important project.
